Position Summary:

The Community Health Worker (CHW) Manager is a non-clinical staff, under the direction of the Nursing Director/Manager. As a role model and leader among peer community health workers, s/he performs CHW functions and manages fellow CHWs in meeting agency goals. Specifically, the CHW Manager coordinates and oversees CHW orientation and on-going trainings. S/he conducts regular meetings to review performance data and provides updates on policy, program, and non-clinical/clinical issues. With leadership principles, s/he mobilizes the team and serves as a liaison with other staff in maintaining effective relationships with providers, support staff, and other community stakeholders to ensure timely and appropriate patient follow up.

KEY FUNCTIONS / RESPONSIBILITIES: The following statements describe the general nature and level of work to be performed by the individual assigned to this job classification.   • Facilitates the ongoing development of internal CHW training tailored to meet the needs of the agency.
  • Coordinates and conducts orientation and on-going trainings to ensure integration of new CHW staff into the agency.
  • Provides support and training for new and veteran CHWs.
  • Explores, researches, proposes and coordinates the implementation of programmatic improvements related to CHWs job functions in the care management process.
  • Fosters collaboration, educates constituencies and, leverages resources to reduce duplication of services.
  • Conducts regular meetings to review relevant performance data and to provide updates on policy, procedures, and other issues.
  • Employing Studer leadership principles, motivates CHWs to follow through on outstanding matters to ensure they are successfully resolved.
  • Represents CCI's interests at internal and external meetings, including the promotion of the CHW role on key coalitions and task forces.
  • Performs periodic practice site visits to participate in Huddles, staff meetings, and update site Triads and/or staff directly of program updates.
  • Under the direction of his/her supervisor, collects and analyzes statistics relevant to care management productivity to ensure adherence to established protocols and benchmarks.
  • Supports and supervises CHWs to ensure they are accurately and consistently documenting activities and plans of action.
  • Utilizes meetings and individual supervision, including monthly rounding, to ensure high quality team work and patient-centered care.
  • Performs chart audits to support quality improvement initiatives.
  • In collaboration with his/her supervisor, assesses training needs of new and veteran CHWs, identifies and coordinates continuing education and staff development opportunities to align CHW skill sets. Verifies adherence to established core competencies and care management protocols.
  • Responsible for Diabetes Self-Management Program coordination.
  • Performs a range of non-clinical care management functions  e. Outreach, health education, informal encouragement/counseling, social support, resource linkages, transportation coordination, and advocacy,  for an active patient caseload
  • Attends assigned site and agency staff meetings, trainings and other meetings as requested.
  • Manages assigned caseload.
  • Other duties as assigned.

 Minimum Qualifications:

  • Bachelor's degree in social services, community health, human services or related fields.
  • Successful completion of a Community Health Worker formal training program or 2 years relevant work experience.
  • Excellent written, oral, and communication skills, including but not limited to listening, ability to articulate difficult concepts, public speaking, team communication and patient advocacy.
  • Experience and ability to work in a multi-cultural setting and on a multidisciplinary team.
  • Ability to initiate and maintain positive working relationships with external organizations.
  • Basic computer skills including Word, Excel, Outlook and PowerPoint.
  • Demonstrated understanding and performance of Community Health Worker core competencies.
  • Demonstrated leadership skills with a minimum of 1 year of management experience.
