Position Summary:

The Family Nurse Practitioner will use his/her experience and clinical knowledge to provide general medical services and patient counseling in collaboration with Community Clinic Physician(s).  Delivers care appropriate for an outpatient, primary care community health center with a multicultural, multilingual patient population.  The Family Nurse Practitioner will report directly to the Health Center Medical Director.

As a member of the health center's integrated care team the Family Nurse Practitioner will fulfill the following KEY FUNCTIONS AND RESPONSIBILITIES towards the provision of patient centered, coordinated patient care:

  • Demonstrates satisfactory knowledge and clinical judgment to provide appropriate care for clinic patients
  • Performs general physical examination as indicated and instructs patients in health promotion and disease prevention.
  • Examines patients to determine presence of illness, utilizing physical findings, laboratory results, medical history, ascertains nature and extent of illness and prescribes treatments as indicated.
  • Documents and maintains patient records of services provided according to agency standards.
  • Develops and institutes appropriate treatment plans.  Treatment will include the care of acute and chronic medical problems.
  • Establishes a collaborative relationship with other medical providers.
  • Participates in continuing education courses, in-services and staff meetings.  Attends other meetings as assigned. 
  • Participates in Performance Improvement activities and Peer Review.
  • Productivity satisfactory and consistent with the agencies goals.
  • Responsible for other related job duties as assigned by the Associate Medical Director.
  • Participates in CCI Transformational activities for becoming a Patient-Centered Medical Home
  • Participates in CCI programs and partnerships as appropriate, which may include the HIP program (Health Integrated Partnership with Family Services, Inc. and Threshold/St. Luke's) and Montgomery County Homeless program


  • Knowledge of the principles and practice of pediatric and adult medicine (including preventive care and acute/chronic disease management).
  • Skill in communicating effectively with patients and their families.
  • Skill in establishing and maintaining effective working relationships with other employees, patients and the general public.
  • Knowledge/experience with Electronic Medical Records (EMR) is a plus.

Minimum Qualifications:

  • Nurse Practitioner certification from an accredited program
  • Active license issued by the State of Maryland Board of Nursing
  • Medical Spanish skills strongly preferred.
  • Computer skills required.
  • Experience working in a multi-cultural setting preferred.
  • Ability to strongly embrace and personify the mission of Community Clinic, Inc. (CCI)
