POSITION SUMMARY

The Behavioral Health Coordinator, at the direction of the Integrated Behavioral Health Director, supports the behavioral health staff and patients at CCI. S/He provides administrative support through patient scheduling, coordinating care with external behavioral health providers, identifying resources for patients and facilitating billing compliance.

KEY FUNCTIONS & RESPONSIBILITIES


1. Participates in coordination of care by assisting with referrals to external mental health and substance abuse providers through the completion of relevant documents and applications for services.
2. Obtains initial authorizations required for Medicaid patients prior to the initiation of therapy or psychiatry appointments.
3. Calls assigned patients who No Show for appointments, particularly those who are high risk behavioral health.
4. Calls patients who fail to return for follow up treatment.
5. Schedules appointments for refugees referred to CCI and schedules appointments.
6. Tracks and notifies behavioral health providers regarding due dates for biannual auditing on Individual Treatment Plans.
7. Assists in identifying resources needed for patients.
8. Assists behavioral health providers with administrative tasks and/or follow up calls.

MINIMUM QUALIFICATIONS


1. Bachelor's degree or three years experience in direct health care contact with clients preferred                  2. Proficient in written and oral communication in English.
3. Excellent communication skills, including but not limited to listening, ability to articulate difficult concepts, public speaking, team communication, telephone skills, and patient advocacy.
4. Experience working in a multi-cultural setting.
5. Ability to work effectively in a multidisciplinary team.
6. Ability to initiate and maintain positive working relationships with external organizations.
7. Understanding of the community served.
8. Basic computer skills including Word, Excel, Outlook and PowerPoint.
9. Bilingual in Spanish (Preferred).
10. Experience with care/case management (Preferred).
