As a member of the operation's team the Call Center Panel Manager will fulfill the following KEY FUNCTIONS AND RESPONSIBILITIES towards the provision of patient centered, coordinated patient care:

  • Utilize patient lists generated by MCOs and other payers to make new patient appointments
  • Point of Contact for appointment setting with community partners
  • Assist with processes to ensure all new MCO patients have access to care within 90 days of enrollment
  • Determine if assigned patients are actively engaged with another medical home and ensure this information is reported back to MCO
  • Use data from multiple sources such as EHR, MCO, HIE, i2i, and others to assist in patient engagement with medical, dental, and mental health management
  • Produce panel reports and gap analysis reports from population health software
  • Produce monthly empanelment report for purposes of calculating capacity and continuity

Minimum Qualifications

  • High school diploma or GED equivalent required; Associate Degree in healthcare related discipline preferred.
  • 1-2 years of experience in an outpatient/community health setting, with knowledge of community health and human services.
  • 1-2 years of experience in health education or related.
  • Bilingual English/Spanish required.
  • Proficient in Microsoft Office Suite
  • Basic understanding of HIPAA and PHI
  • Knowledge of medical terminology
  • Basic navigation knowledge of electronic medical record applications such as CPS 08
